Applying for a Medical Marijuana Registry Identification Card

Once you and your physician have determined that you are qualified and you have decided to apply for a Medical Marijuana Registry card, you may request an application packet from the Colorado Department of Public Health and Environment (CDPHE) online at:

Upon receiving your application packet,

1.       Complete the entire Application for Identification Card application form. Sign and date it.

You may choose a caregiver. A caregiver is defined as “a person, other than the patient and the patient's physician, who is eighteen years of age or older and has significant responsibility for managing the well-being of a patient who has a debilitating medical condition.” For example, we, at The Kind Room, are primary caregivers.

2.       Ask your physician to complete, date and sign the Physician Certification Form. If they do not agree to sign the form, contact The Kind Room for further instructions.

3.       Make a copy of your photo identification that establishes Colorado residency (such as a driver’s license).

4.       A non-refundable $90.00 application fee (check or money order payable to CDPHE) is required.

5.   Send completed application packet and application fee by USPS certified mail for proof of mailing.

 

Make sure that all forms/documents are complete to avoid delays. Keep copies of these forms/documents for your records.

The information you provide will be verified within 30 days of receiving all the application materials. Expect to receive your Medical Marijuana Registry card in three to four weeks.

 

You will receive one card with the patient’s (your) information and caregiver information, if designated. The caregiver will not receive a card. Make a photocopy of your Medical Marijuana Registry card and give to your caregiver.

Plan for Your Medical Requirements

It will take a patient or a caregiver 3-6 months to grow the first crop. Outdoor gardens will only produce one harvest each year. Indoor gardens can be subject to crop failures for a variety of reasons. The limit of three flowering plants means there is no margin for error.

 

Patients and caregivers should expect periods when they will not have medicine and should discuss some contingency plans. Being a member of The Kind Room cooperative can provide a safety net for the acquisition of medicine and can be utilized as a full time primary caregiver.

Terminating the Relationship

The patient/caregiver relationship can be terminated at any time. However, termination puts the caregiver in a precarious situation. They will have an excess of the amounts allowed and are no longer exempt from the criminal laws of Colorado. Advance notice to your caregiver is helpful.
